### How to Build a Defense Factory: OpenAI's Playbook, Read by Engineers Who Build Agents

In September OpenAI published the Defense Factory playbook: an agent-first operation that connects your security tools, skills and reproducible environments to fix vulnerabilities continuously. It explains what to build and gives a six-week plan, and it leaves the engineering to you. We build agent systems for a living, so here is what each part of the playbook means in practice, where the work actually is, and how to keep the agents themselves from becoming the next incident.

Price per million tokens tells you very little about your bill. We priced three real task shapes, a RAG support answer, an eight-call agent run and a document extraction, on ten current models from OpenAI, Anthropic and Google, with and without prompt caching and batch. The model tier you pick moves the cost a hundredfold. Caching and batch move it by a quarter to a half.

On October 21, PHP shipped emergency patches for two unauthenticated remote code execution bugs that Palo Alto Networks counts on more than 22 million exposed instances. Earlier this year a backdoored LiteLLM reached 47,000 installs in three hours, a prompt injection in a GitHub issue title turned into a tampered Cline release on 4,000 machines, and the UK AI Security Institute reported an agent that opened a malicious pull request on a real open-source project and invented identities to get it merged. They are one story: an agent inherits every weakness of the stack it runs on, and it adds inputs that no existing security tool was built to inspect. Here is what that means for teams putting agents into production, and the controls that have held up in ours.

### OpenAI's Decisions API: A Fast Classifier for Agent Routing, and the Calibration Test You Have to Run Yourself

OpenAI's Decisions API went into public beta on October 6. It answers bounded questions (is this true, which of these, how severe) in about a tenth of the time of a normal model call, at $0.10 per million input tokens. It is the right shape for the routing and gating steps inside an agent. We have moved the routing classification of a production multi-agent system onto it and the gain on that step was clear. Early testers also found that its choice answers are not calibrated, that answer order moves the probabilities, and that there is no caching yet. Here is what shipped, where it fits, and what to test before you put it in a production loop.

Amazon Bedrock AgentCore is the first AWS primitive built for the actual shape of an agent workload: long, stateful, bursty, and full of privileged tool calls. After running multi-agent systems on it in production since its preview, here is an honest account of what it does well, where the model leaks, and the gotchas around sessions, headers, cold starts, and quotas that cost real engineering weeks.

### Your Voice Input Hallucinates on Silence. Here Is the Gate That Stops It.

Speech-to-text models always return text, even when the microphone captured nothing. Worse, if you biased the transcription with a domain glossary for accuracy, the fabrication comes back written in your product's own vocabulary. This is the two-layer gate we shipped to stop it: a Silero VAD check before the model is called, and a logprob backstop after it, with the parameters, the trade-offs, and the two things we got wrong on the first pass.
